Kathleen J. Jackson

Dr. Kathleen Jackson has a broad background as a family nurse practitioner in primary care, with specific knowledge and experience in prevention and management of chronic disease.  Dr. Jackson delivers healthcare to vulnerable populations, working with communities to improve health (community engagement). Previously working with Jeffrey Brenner and the Camden Coalition of Health Care Providers (CCHP) in Camden, N.J., Dr. Jackson managed an interprofessional team for the care of, and developed strategies for, self-management of chronic conditions in socially complex patients. Working with CCHP and the Camden Citywide Diabetes Collaborative she developed and implemented strategies in transforming primary care practices to more effectively focus on care of patients with chronic conditions and complex social issues. Working in low income Camden neighborhoods for approximately 10 years, Dr. Jackson engages with communities for health, providing preventative health screenings, vaccinations, health education, and support for community capacity building. For example, Dr. Jackson has promoted partnerships to bring fresh produce into several low-income communities and connected public housing communities with legal aid for the residents to form non-profits to raise funds for their community. 

Dr. Jackson is a board-certified Adult and Family Nurse Practitioner with more than forty years of experience in the healthcare field teaching nurse practitioner students for over 20 years. Her scholarly work focuses on the social determinants of health and engaging communities to promote the wellbeing of families. She received and AANP “Take Care of Health” grant to engage residents and stakeholders from two low income communities to discuss their thoughts on health and experiences of healthcare. More recently she received three competitive grants from Rutgers University Camden, two from the Provost Fund for Research, a Catalyst Grant for “Being Mindful: Improving Health in Vulnerable Communities” , a Mutidisciplinary Grant for “Family Members as Coaches for Patients with Diabetes,” and  an Experiential Infusion Grant.

Dr. Jackson serves as a Clinical Assistant Professor of Nursing at Rutgers School of Nursing–Camden; she holds years of expertise and experience in family primary healthcare, facilitating transformation and leading interprofessional care of patients and community engagement, as well as teaching in graduate and interprofessional education.
